If so, we want you! EdenHospice at EdenHill Communities is looking for a Qualified

Hospice RN Case Manager! EdenHill Communities is a faith-based, not-for-profit senior

living community and the only Life Plan Community (Continuing Care Retirement

Community) in the New Braunfels area. From independent living with resort-style

amenities to quality assisted living, memory care, skilled nursing, and rehabilitation, we’ve

been committed to enriched retirement living for more than 116 years. Our full continuum

of care is located on a nature-rich campus of 27-acres in Texas Hill Country.

strive to provide our residents with services designed to achieve quality care and quality of

We are currently seeking a motivated Full-time RN Case Manager to join our hospice team.

Job Description/ Duties

Essential Duties and Responsibilities include the following:

  • Perform evaluations for hospice care along with informational visits as needed

Including admission and consent as necessary.

  • Coordinates hospice care by working with other IDG members as well as with the

patients/family members to ensure needs are met. Attend IDG meetings.

  • Evaluate the effectiveness of hospice services for the patient and family on an

ongoing basis

  • Utilize the nursing process to provide professional nursing care. Coordinate Patient

care with the attending physician and Hospice Medical Director and NP.

  • Manage and update patient plan of care and medications reconciliations.
  • Continually strive to improve his/her nursing care skills by attending in-service

education, through formal education, attendance at workshops, conferences,

active participation in professional and related organizations and/or individual

research and reading. Complete assigned education throughout the year.

  • Provide education to patients/family members regarding the dying process, plan of

care and offer guidance with healthcare decisions.

  • Perform death visit per agency’s policy.
  • Participate in the patient’s discharge planning process.
  • Adhere to Federal, state and accreditation requirements including Medicare and

Medicaid regulations.

  • Participate in the Quality and process improvement program
  • On call duties in a rotation to include a few weekend days
  • Holiday on call rotation
  • Other duties as assigned

  • Mileage paid for travel per IRS guidelines.

Pay: $33.00 - $38.00 per hour
